c) Proof of purchase: (any of the following)
- A scanned copy or photo of any 2 prepaid cards used to top up the account showing the serial and pin number visibly;
- Screenshot of the receipt from any payment portal e.g. MOL, Paypal, etc.
- Or alternatively you may fill out the Account Retrieval Form.
